Questions

Is well-formed the same as valid?

No. Well-formed means the syntax is correct (tags close, attributes are quoted). Valid means it also follows a schema. Use the XSD validator for that.

Are external entities loaded?

No. DOCTYPE entities and external resources are never fetched or expanded.
